I suspect (we're talking about Doctor Who, right?) that the Gallifreans have another language based not on sounds but on pheremones, like ants do. They probably have a sort of back-up language based on sounds to communicate with humans. So the Doctor may have a real name that is a smell. Which could be interesting when it comes to all the body regenerations: I wonder if they need to adopt a new name to match every new body odor?
Or maybe, since the good Doctor always seems to understand any alien language effortlessly, that it's based on a telepathic system, with the spoken word only there as an adjunct (again to help the hapless humans to understand.)
