I am not referring to difficulty in making choices but rather difficulty in processing multiple inputs.  For example, if I have 3 different homework assignments, I haev difficulty starting on any one of them, because if I do all I will be able to think about is the other 2 im not doing.  Or on a more fundamental level, If I am watching something on television and a person is talking in the same room, no matter how quiet they are I will not be able to understand a word of what is being said on TV, because I cant process the two inputs simultaneously.
With ADHD, it is not quite so simple.  I have the attention span of a squirrel on methamphetamines.  I can't stay in one state of mind for more than a few minutes, in most cases.  This makes it a bit dificult to stay consistant.

A useful analogy here is that most people have broadband internet connnections in their minds but we aspies have dial-up instead.
