As part of a medical questionnaire with my employment contract, one of the questions asks "have you ever seen a psychiatrist,  or had an emotional or psychological disorder or stress-related problems?"

Surely I don't need to disclose on the form that I visited a psychiatrist when I was still a child (16 years old), about Asperger syndrome.  I was never formally diagnosed anyway, but my parents and I thought I have it, and the psychiatrist did not dispute this, although he did not give a formal diagnosis.  I am now 21 years old (nearly 22).
I have never suffered from depression or stress-related problems.
I don't see why you should disclose this information.  I think by 'seeing a psychologist' they mean you went there to sit on "The Couch", if you know what I mean.

Anyway, when you did visit the psychologist you were, in the eyes of the law, still a minor.  Employers aren't usually interested in events that occured before you were 18.
As its a contract, I think you need to be honest, even though its not really fair.
It doesnt say as an adult, it says 'have you ever' seen one.

If you arent honest in a contract, it can cause problems for you later on in a job.
Depending on where you live or the type of work you are applying for, this question might be illegal to ask in that situation.

If you are applying for an example, a military spy service or some similar sensitive job, it might be important to disclose.  If you are just applying for a regular job at a store, then it is none of their business.  Anyway, how would they find out about your medical history?
I think that question is too intrusive.
